PerformFlow is a Google Forms add-on which automates approval workflow right on Google Forms and with Gmail.
Users can use Google Forms to create request form (e.g. Leave Request). Their respondents will submit request via that created form, then specified recipients will receive approval email. Those recipients can make approval decision right on that email.
