The Future of Patient Journey is here!

What do you like best?

The way Salesforce's Health Cloud connects the care plans to recovery plan dates and the view of a Patient's journy via timeline and also the view that was used to map a patient's circle. These easy to see dashboard/report kind of tools help in assessing a patient's risk level at a glance. I had previously worked on a custom solution for a Patient Onboarding and Adherence for a Big Pharma company - we had to do a lot of custom work to get what we desired but with health cloud more than 70 % of what is required is built in as a standard. The risk segmentation is one of the core features which has been implemented in a very organised manner.

What do you dislike?

The care plan templates can be better, they can do better mapping to Problem/Goal vs time scale. I would say some of the features are obsolete or not valid in a health case scenario however that is what you get when the software wants to be a jack of all trades instead of specializing in one - but in the context of heath cloud it actually works well in it's favor. I would say most of the smaller dislikes I had like the association between a pharmacy and physician and the benefits investigation module have already been fixed.

Recommendations to others considering the product:

Please plan your implementations before hand and do a scrutiny of data points - integrations which are complex need to be at the top of your list. Consider the limitations of data and data types in integration requirements.

What problems are you solving with the product? What benefits have you realized?

When I worked on the health cloud we built multiple risk segmentation modules and patient survey modules which are dynamically assigned based on the initial assessment. From our data we were able to analyze that using this model increased the patient adherence levels when we used the risk segmentation and survey results in conjunction.
